Community Data

Community Health Solutions has posted an updated set of Community Indicators in the dropdown menu under Resources.  This is a beta test for which we welcome feedback from CBHI Provider Network Members. If you have any feedback in the form of corrections, suggestions, or requests, please click here to let us know.

The CBHI Community Data Portal has been updated with the most recently available indicators for 2013 and 2012.  Select Community Indicators in the Resources dropdown menu to find indicators that can be used to support strategy development, program planning, and resource development.
