The Center for Health Care Strategies, Inc. (CHCS) has developed a brief for the Centers for Medicare & Medicaid Services’ Health Home Information Resource Center that highlights health home opportunities for children with SED. The brief also presents factors for states to consider as they develop models that address this population’s unique needs.
